Suprachoroidal hemorrhage (SH) is a rare complication of scleral-fixated intraocular lens (SFIOL). In this series, we report three cases of sutureless SFIOL who developed SH postoperatively. Details of surgery, treatment, and outcome were analyzed. Two cases had limited SH which resolved with conservative treatment and had optimum visual recovery. One case developed massive SH needing surgical drainage, with final best-corrected visual acuity of 6/18 due to a persistent retinal fold. SH can be a potential complication while creating scleral grooves for haptic fixation. The use of a bent needle instead of a straight microvitreoretinal knife while creating scleral grooves might reduce the risk of SH in sutureless SFIOL surgeries.

PURPOSE: The purpose of this study was to evaluate the usefulness and assess the risks associated with occlusive contact lenses (OCLs) use to treat refractory amblyopia in children who failed the conventional amblyopia treatment with patching and atropine penalization. METHODS: This was a retrospective case series of all children treated with an OCL in the sound eye over an 8-year period. Data were collected on the age, gender, and type of amblyopia, previous treatment received, and its duration. The compliance, tolerance, and adverse effects of OCL were recorded. RESULTS: There were 12 patients of which 10 were male. The mean age at the time of OCL use was 47.3 months (range 26–86 months). Anisometropic amblyopia was present in 1, combined anisometropia and strabismus in 3, strabismic amblyopia in 5, and stimulus deprivation amblyopia in 3. All patients had conventional patching for a mean period of 14 months, followed by atropine penalization in nine patients for a mean period of 4 months. The mean duration of OCL use ranged from 2 weeks to 6 months. There were nine patients with visual improvement (75%). The mean improvement seen was 0.4 log of the minimum angle of resolution. There was no occlusion amblyopia. Four patients had conjunctivitis; one had a peripheral corneal abrasion. CONCLUSION: The OCL is a useful adjunct in the treatment of amblyopia that is refractory to conventional treatment. The side effect profile is acceptable, but patients require close monitoring.

We are reporting four accounts of rhino-orbital mucormycosis in patients during and after recovery from SARS-CoV-2 infection. The patients were diagnosed and treated for COVID-19 according to the current treatment protocols, following which they presented with sudden proptosis, ophthalmoplegia, and conjunctival injection, confirmed by magnetic resonance imaging and histopathological examination. The patients were treated with intravenous liposomal amphotericin B, and the outcome was observed. Early diagnosis and prompt intervention can substantially reduce the morbidity and mortality rates in these patients.

PURPOSE: The objective of this study was to assess the concordance between the values obtained in measuring central corneal thickness using the OrbscanIIz^® and the contact ultrasonic pachymeter available in our public ophthalmology service. METHODS: Measurements were taken from 88 eyes of 44 patients using the two instruments. The data obtained were statistically analyzed using version 22 of the IBM SPSS^® program. RESULTS: The mean of central corneal thickness measurements obtained from OrbscanIIz^® was significantly higher than that obtained from ultrasound pachymetry. However, the mean of differences between both instruments was only 7.22 μ, which could be considered a clinically insignificant result when considering the good concordance obtained between both systems. CONCLUSION: OrbscanIIz^® and ultrasound pachymetry can be interchangeable in the usual public clinical practice when measuring central corneal thickness. This is the first research found in the literature that uses a concordance study to compare the data resulting from central corneal thickness measurements obtained by an OrbscanIIz^® and an OcuScan^® pachymeter in our environment.

PURPOSE: The purpose of this study was to compare ultrasound pachymetry Ultrasound pachymetry (UP), specular microscopy (SM), and anterior segment optical coherence tomography (ASOCT) in the measurement of central corneal thickness (CCT) in cases of corneal edema before and after uncomplicated phacoemulsification. METHODS: Preoperative pachymetric measurements were performed in all patients before cataract surgery. Sixty -seven patients who developed corneal edema after routine faoemulsification surgery were included in the study. CCT measurements were made the day before surgery and on the 1^st, 7^th, and 14^th days after surgery. Correlation and agreement between the devices were quantified with the intraclass correlation coefficient (ICC), limits of agreement, and Bland–Altman plots. RESULTS: The mean age of the patients was 70.99 ± 9.56 (36–92) years. While CCT could be measured in all eyes with ASOCT after surgery, the number of eyes measured with UP and SM were 24 (11.7%) and 27 (16.1%), 55 (82.1%) and 52 (77.6%), and 46 (88.8%) and 45 (86.9%) on postoperative days 1, 7, and 14, respectively. ICC values between UP and SM, UP and ASOCT, and SM and ASOCT, respectively, were: preoperative 0.91, 0.94, and 0.92; 1^st postoperative day 0.93, 0.86, and 0.93; 7^th postoperative day 0.94, 0.85, and 0.96; and 14^th postoperative day 0.95, 0.92, and 0.96. CONCLUSION: In all eyes with corneal edema, CCT measurements can be taken with ASOCT; however, in some cases, CCT measurements with SM and UP cannot be taken. However, there is a good correlation between measurements taken with these devices.

PURPOSE: The purpose of the study was to analyze the demographics, visual acuity (VA), etiologies, recommended low vision assistive products (LVAP), and the acceptance rates of LVAP in various age groups. METHODS: This was a long-term retrospective review of all the patients presenting to the low vision clinic of our tertiary eye care hospital from January 2011 to December 2016. Data obtained included age, gender, VA, visual fields, ocular pathology causing the low vision, and types of LVAP advised. The primary outcome was to analyze the type of LVAP prescribed in different age groups, and the secondary outcome was the acceptance rate of LVAP. RESULTS: We analyzed the results of 8309 patients, out of which 2844 (34%. 2) were <15 years of age, 2425 (29.5%) were between 16-40 years, and 3013 (36.3%) were above 40 years. A total of 5522 (66.4%) had best-corrected visual acuity (BCVA) ranging from 6/18-3/60, and 2796 (33.6%) had BCVA from 3/60-No PL. Approximately 38% improved with LVAPs. The most common etiology was retinitis pigmentosa in 1545 (18.6%) patients, followed by congenital nystagmus in 1482 (17.8%), and the least was albinism 383 (4.6%). Maximum prescribed and accepted LVAP were hand and stand magnifiers among 1017 (44.3%) and 512 (52.6%) patients, respectively. CONCLUSION: Products that are easy to use, require lesser adaptability, are cheap, and require lower maintenance have maximum acceptance rates. We suggest that great emphasis should be laid on training, education, and guidance for low vision rehabilitation centers.

A 24-year-old woman presented for routine clinical evaluation. Her best-corrected visual acuity was 20/20 and slit-lamp examination of the anterior segment was normal in both eyes. Fundus examination of the left eye revealed a hypopigmented lesion, one disc diameter temporal to the fovea, with a hyperpigmented tail extending temporally from the main lesion, consistent with a torpedo maculopathy. Fundus autofluorescence showed an hypoautofluorescence with hyperautofluorescent border. Swept-source optical coherence tomography (OCT) showed a normal inner retina and a degeneration of the outer retina without retinal cavitation. OCT angiography (OCT-A) of the choriocapillaris layer revealed reverse shadowing caused by the increased transmission from the atrophied outer retina and RPE within the torpedo lesion and attenuation of signal in the area of the temporal hyperpigmentation. The superficial capillary plexus was normal. This case includes a multimodal imaging with OCT-A of torpedo maculopathy in a Tunisian woman. Further reports are required to provide a better understanding of this rare condition.

PURPOSE: To evaluate the learning curve of continuous curvilinear capsulorhexis (CCC) and to assess the number of surgeries required to master it among residents in a postgraduate teaching institute. METHODS: The present prospective observational study was based on the completion time and complication rates related to CCC performed using various techniques by 10 students in the 2^nd (JR2) and 3^rd-year (JR3) of residency. CCC was performed either by a cystotome or capsulorhexis forceps or by a combined method in 253 eyes, of which 160 eyes (63.2%) were operated by JR3 and 93 (36.8%) by JR2. The complication rates were studied with respect to the number of capsular extensions, posterior capsular rent (PCR), zonular dehiscence, need for senior surgical assistance, and nucleus drop. RESULTS: The average time required for the completion of CCC was 412 ± 90.5 s. The average number of times residents required to fill the anterior chamber with viscoelastic was 6.9 ± 1.4. The average size of CCC was 7 ± 0.66 mm. Extended CCC was the most common complication. JR2 required assistance from a senior surgeon in 47 eyes (50.5%), whereas JR3 required assistance in 39 eyes (24.4%) (P = 0.0001). The rate of PCR was not significantly different in JR2 (7.5%) and JR3 (8.8%). CONCLUSION: CCC is a difficult step to master in the trainee. Focusing and practicing on this step will help to reduce the complications and maximize proficiency. Approximately 6–eight surgeries are required to master CCC.

PURPOSE: The purpose of this study was to determine the demographic and clinical patterns of optic neuritis (ON) in patients presenting to a tertiary health-care institute and to study the incidence of multiple sclerosis (MS), magnetic resonance imaging features in varied ON, treatment outcome, and prognosis. METHODS: A retrospective analysis of patients with first episode of ON presenting to a tertiary care center during the period from March 2013 to March 2021 was done. Details of ocular examination were retrieved from medical records and statistically analyzed. RESULTS: Three hundred and fifty-four participants with ON were included in this study. The mean age was 40.25 ± 12.2 years. The male: female ratio was 1:1.35. 48.1% had visual acuity of <3/60. Based on clinical presentation, papillitis was seen in 31.5% of subjects, neuroretinitis in 24.1%, and retrobulbar neuritis in 44.4%. Based on etiology, 79.6% were idiopathic, 1.8% presented with infectious ON, and 9.26% were associated with demyelinating disease (MS). CONCLUSION: Females were predominantly affected. Idiopathic ON formed the major subset etiologically. Sixty-six percent had visual recovery of 6/18 or better following corticosteroid therapy. 9.2% revealed multiple intracranial lesions on neuroimaging, suggesting high association with MS. Therefore, early diagnosis, vigilant monitoring of steroid therapy, and regular follow-up screening for MS remain the mainstay of management in ON.

Teleophthalmology is the provision of ophthalmic care using digital medical equipment and telecommunications technology. While teleophthalmology has been practiced since the 1990s, it became a crucial medical service during the COVID-19 pandemic when doctors were required to make diagnoses from a distance. This was certainly the case in Saudi Arabia, a relatively large country with limited medical workforce to meet demands, yet a high prevalence of diseases such as diabetes, glaucoma, and cataracts which are considered leading causes of visual impairment. In response to the pandemic, Saudi Arabia's Ministry of Health issued scientific instruction manuals and guidelines for health-care providers which paved the way for increased use of digital technologies and solutions to mitigate the health and economic impacts of COVID-19. The pandemic served to accelerate the use of virtual ophthalmology in Saudi Arabia with the most common teleophthalmology applications being teleconsultation and disease-specific applications such as diabetic retinopathy and retinopathy of prematurity. Although some challenges relating to implementing teleophthalmology in Saudi Arabia remain, it is felt that there are great opportunities to expand the use of telemedicine in ophthalmology in this country.

PURPOSE: The objective of this study on patients with albinism in different age groups was to compare their level of visual impairment with the low-vision intervention (LVI) and its benefit. METHODS: The medical records of 72 patients with low vision secondary to albinism who were referred to the low vision care clinic from 2015 to 2017 were analyzed. This included the demographic profile such as age, gender, occupation, ocular history, visual acuity status, and type of low-vision device (LVD) preferred. The LVDs prescribed and its subsequent improvement was compared. RESULTS: In this data, 70 (97.2%) people had oculocutaneous albinism and 2 (2.8%) had ocular albinism. Majority of the patients had hyperopic astigmatism 42 (58.3%) and with-the-rule astigmatism 58 (93.5%). Presenting mean visual acuity for distance was noted to be 0.88 logMAR which improved to 0.83 logMAR with the help of spectacle correction. The most commonly prescribed LVD was a dome magnifier for 15 (34.9%) patients. In all the patients, there was statistically significant improvement (P < 0.05) in near vision with the help of LVDs. CONCLUSION: The study highlights the importance of appropriate LVI for each subdivided age group. Patients with albinism who have received medical and surgical treatment have no or a limited role in restoring useful vision.

A 39-year-old woman, without any systemic or ocular history, presented with a paracentral scotoma in her right eye with normal visual acuity. Humphrey's visual fields showed a reversible enlargement of the right blind spot. The final diagnosis was acute idiopathic blind spot enlargement which was successfully treated with intravenous steroids. Although differential diagnosis of blind spot enlargement can be challenging, multimodal imaging with combination of visual field's assessment can help us make the right diagnosis.

PURPOSE: The purpose is to study the change in visual acuity obtained with sclerocorneal contact lens (SCL) in patients with irregular corneal astigmatism. METHODS: A prospective pre–post observational study was designed to include consecutive consenting patients with irregular corneal astigmatism and best-corrected visual acuity worse than 6/12 and fitted with SCL. Visual acuity was assessed using Snellen charts before fitting SCL and 1 week after the fit and converted to the equivalent logMAR units for analysis. RESULTS: We included 51 eyes of 41 patients with a mean age of 26.31 ± 8.86 of which 32 (78%) were men. Keratoconus was the most common indication (n = 42 eyes, 82.35%), followed by corneal scar (n = 3, 4%), post keratoplasty (n = 2, 4%), high myopia (n = 2, 4%), pellucid marginal degeneration (n = 1, 2%), and aphakia (n = 1, 2%). The mean overall uncorrected visual acuity improved significantly (P < 0.001) with SCL from 1.18 ± 0.34 logMAR (6/120 Snellen's equivalent) to 0.27 ± 0.15 (6/9 Snellen's equivalent) at 1-week post-SCL fitting. The mean overall best spectacle-corrected visual acuity improved from 0.89 ± 0.45 logMAR (6/36 Snellen's equivalent) to 0.26 ± 0.15 (6/9 Snellen's equivalent) at 1-week post-SCL fitting. CONCLUSION: Sclerocorneal contact lens improves visual acuity significantly in patients with irregular corneal astigmatism and reduces the need for corneal transplantation.

PURPOSE: Idiopathic intracranial hypertension (IIH) is a disorder with elevated intracranial pressure more than 250 mm H2O, without evidence of meningeal inflammation, space-occupying lesion, or venous thrombosis. In this study, we aim to study the clinical profile, evaluation, management, and visual outcome in a hospital-based population of IIH cases in Jeddah. METHODS: This is a retrospective observational cohort study that included the medical records of all patients referred to neuro-ophthalmology service for evaluation of papilledema. The medical records have been reviewed from October 2018 to February 2020 at Jeddah Eye Hospital, Saudi Arabia. A total of 51 patients presented with papilledema in the studied period. Forty-seven patients met our inclusion criteria and were included in the study. RESULTS: The study found that the incidence rate of IIH is 16:100 of the referred cases to the neuro-ophthalmology clinic. Most of the patients were females (41, 91.2%), with a mean age of presentation of 32 ± 11 years. The most common presenting symptom was headache (40 patients, 88.8%), followed by transient visual obscuration (TVO) (20 patients, 44.4%), and reduced visual acuity (15 patients, 33.3%). All 45 patients were started on medical treatment with oral acetazolamide with four patients (8.8%) shifted to topiramate because of the lack of response or intolerance to acetazolamide while four patients (8.8%) underwent lumbar-peritoneal shunt because of inadequate control of the disease despite the treatment with medical therapy. For both eyes, the change in visual acuity across all assessment points was statistically significant. Nevertheless, there were no significant changes in the visual field findings among all of the compared assessment points. CONCLUSION: The present study has shown that IIH-related papilledema is common in young female patients with headaches, TVOs, and reduced visual acuity. Those are the most common symptoms in our IIH population. Medical treatment and monitoring of IIH is efficient and should be considered to enhance the prognosis of IIH-related complications. In addition, the visual acuity and the visual field should be frequently monitored for these patients.

PURPOSE: Improved stereoacuity following delayed strabismus surgery is associated with long-term alignment of eyes and reduced severity of amblyopia. The objective of this study is to evaluate stereopsis following delayed strabismus surgery in early-onset strabismus. METHODS: It is a hospital-based prospective, nonrandomized, interventional case study. Thirty patients with early-onset strabismus (before 3 years of age), presenting after 6 years of age were included. History taking and full orthoptic workup was done. Stereopsis was tested with Titmus Fly Chart test. The patients were treated surgically to correct strabismus and were followed up for 3 months. Quantitative parameters were compared between preoperative and postoperative follow-up at 3 months using McNemar test, Chi-square test, and independent t-test. RESULTS: Of the 21 patients who had gross stereopsis preoperatively, nine of them (42.86%) attained fine stereopsis at 3 months postoperatively (P = 0.003, McNemar test). Ten (33.33%) of 30 patients had amblyopia and three out of 30 patients (10%) had preoperative “nil” stereopsis. Of the patients who showed improvement (16 of 30 patients – 53.33%), most of the improvement was observed in (a) exodeviations (56.25%, n = 9), (b) earlier age of presentation-before 10 years of age (47.36%, n = 9). CONCLUSION: The benefits of surgical correction of early-onset strabismus include improvement in stereopsis. Therefore, even in an early-onset squint with a delayed presentation rehabilitation of stereopsis should be the goal of treatment.

Graves' ophthalmopathy is the most common cause of both unilateral and bilateral proptoses in adults. Peripheral primitive neuroectodermal tumor (pPNET) is a small round cell malignant lesion of neuroectodermal origin which very rarely affects the orbit. In this case report, we have discussed about a young woman with existing Graves' ophthalmopathy who presented with worsening proptosis; computed tomography imaging revealed an irregular mass lesion in the right orbit without bone erosion. Biopsy and immunohistochemistry of the mass lesion revealed features of primitive neuroectodermal tumor (PNET). The tumor was MIC-2 gene positive, and on follow-up, no recurrence was noted after successful surgical resection. PNET of the orbit is very rare, and to our best knowledge, this is the first case of peripheral PNET in patent with Graves' ophthalmopathy.
